• - On October 2, 1996, a Line A train "Line A (Medellín Metro)") derailed while making return maneuvers at Poblado Station "Poblado Station (Medellín Metro)"), no injuries were reported and the train was taken to the workshops shortly after to be inspected and repaired.[42].
• - On January 12, 2014, a retaining wall on the left bank of the Medellín River between the Ayurá stations "Estación Ayurá (Medellín Metro)") and Envigado "Estación Envigado (Medellín Metro)") of Line A "Line A (Medellín Metro)") collapsed due to high rainfall and lack of maintenance in the pipeline, the service was suspended between the stations Aguacatala "Aguacatala Station (Medellín Metro)") and La Estrella "La Estrella Station (Medellín Metro)") for 10 days.[43].
• - On March 13, 2014, there was again a landslide on a retaining wall of the Medellín River between the Aguacatala stations "Estación Aguacatala (Medellín Metro)") and Ayurá "Estación Ayurá (Medellín Metro)") of Line A "Line A (Medellín Metro)"), paralyzing 6 stations in the south and causing chaos in mobility, the service was suspended for 11 days.[44].
• - In the early hours of July 22, 2018, a group of five graffiti artists who were vandalizing a train parked between the Poblado stations "Estación Poblado (Medellín Metro)") and Aguacatala "Estación Aguacatala (Medellín Metro)"), were run over by a train carrying out inspections prior to the start of commercial service, three people died, two were unharmed and escaped from the scene.[45].
• - On January 7, 2020, a cable that was being mounted on the pylons of the new Metrocable Line P "Line P (Metrocable de Medellín)"), fell on a gas station and the Autopista Norte "Autopista Norte (Medellín)"), leaving 7 people injured and considerable structural damage to the service station.[46].
• - On March 21, 2021, due to heavy rains, a landslide occurred next to the railway between the Acevedo stations "Estación Acevedo (SITVA)") and Madera "Estación Madera (Medellín Metro)"), causing delays in the service, Line A "Line A (Medellín Metro)") operated normally between La Estrella "La Estrella Station (Medellín Metro)" Medellín)") and Acevedo "Estación Acevedo (SITVA)"), while the remaining section towards Niquía "Estación Niquía (Medellín Metro)") operated with trains every 15 minutes and supported by Metroplús standard buses. Line A returned to operating normally until April 5. [47].
• - On June 16, 2022, due to heavy rains, a sinkhole was generated next to the railway between the Caribe stations "Estación Caribe (Medellín Metro)") and Tricentenario "Estación Tricentenario (Medellín Metro)"), separating Line A "Line A (Medellín Metro)") into two, trains operated between Niquía "Niquía Station (Medellín Metro)" Medellín)") - Acevedo "Estación Acevedo (SITVA)") and Caribe "Estación Caribe (Medellín Metro)") - La Estrella "Estación La Estrella (Medellín Metro)") with reduced frequencies, and requiring transfers to metro buses to make the complete trip, causing chaos in the mobility of the Metropolitan Area for a week.[48].
• - On February 3, 2022, two metro workers were hit by a train at Sabaneta Station "Sabaneta Station (Medellín Metro)") while carrying out routine inspections on the tracks.[49].
• - On August 28, 2023, a technical failure in a train leaving the Industriales Station "Estación Industriales (Medellín Metro)") during the afternoon rush hour caused chaos when passengers along Line A "Line A (Medellín Metro)") decided to walk along the tracks, ignoring the instructions of the operational staff, this caused service on Lines A "Line A (Medellín Metro)") and B "Line B (Medellín Metro)") were suspended indefinitely, along with Metrocable lines K "Line K (Medellín Metrocable)"), P "Line P (Medellín Metrocable)") and J "Line J (Medellín Metrocable)") until safe operation is guaranteed. The service resumed at 8:18 p.m.[50].
• - On November 2, 2023, two Line T trams collided with a public service bus and two motorcycles near the San José Stop "San José Station (Medellín Metro)"), 15 people were injured. According to witnesses, the tram that was heading Oriente-San Antonio omitted a stop sign.[51].
• - On June 26, 2024, a collision was recorded between two cable cars of the Metrocable Line K "Line K (Metrocable de Medellín)") on the eastern square of the Popular Station "Estación Popular (Metrocable de Medellín)"), which resulted in the lifting and immediate fall of one of the cabins onto the public road, in which 11 passengers were traveling. The accident left 20 people injured and one dead.[52].
• - On October 21, 2024, an incident occurred on Line C "Line C (Medellín Metro)") (or link line) after a short circuit occurred in the catenary while a train without service was passing, causing damage and the suspension of service on Line B "Line B (Medellín Metro)"), and reduced service on Line A "Line A (Medellín Metro)"), generating chaos in mobility for 4 hours. Hypotheses suggest that lightning struck the area during the weekend, or even on the same day of the incident. [53].
• - On April 20, 2025, damage was recorded in the catenary "Catenaria (railroad)") in the afternoon, near the Universidad station "Estación Universidad (Medellín Metro)"), due to a lightning strike that fell in the area, which forced the service of Line A "Line A (Medellín Metro)") to be provided only between Prado "Estación Prado (Medellín Metro)") and La Estrella "La Estrella Station (Medellín Metro)") for the rest of the day while the incident was resolved.[54] Full service on line A was restored early on Monday, April 21.[55].
• - On October 20, 2025, due to heavy rains, a sinkhole was generated in the railway between the stations Poblado "Estación Poblado (Medellín Metro)") and Aguacatala "Estación Aguacatala (Medellín Metro)"). Due to this, Line A "Line A (Medellín Metro)") operated between Niquía "Niquía Station (Medellín Metro)") - Poblado "Poblado Station (Medellín Metro)") and Aguacatala "Aguacatala Station (Medellín Metro)") - La Estrella "La Estrella Station (Medellín Metro)"), while the affected section (Poblado - Aguacatala) was supported by buses Metroplús registers. The integrated routes of the southern municipalities were integrating into the Poblado station, causing chaos in mobility.[56] Line A returned to operating normally as of October 26. [57].
